If you would like assistance in making a vaccination appointment, fill out this form. Someone from the city government will follow up with you in the next week about setting up an appointment. Given the limited supply, they cannot guarantee an appointment at a specific location. But they will do their best to find a location with availability that works for you.

The COVID-19 vaccines that are now rolling out are a miracle of modern science and one of the best hopes we have of ending this pandemic soon. But there are a lot of questions, concerns, and even fears about the vaccines. So the Jamaica 1st Ward of The Church of Jesus Christ of Latter-day Saints has invited a public-health expert to provide factual, science-based information about the vaccine and answer your questions.
- Learn more about the COVID-19 vaccines currently available and how they were developed.
- Find out what the vaccination effort means for you, your loved ones, and our community.
- Get information on how to make an appointment to receive the vaccine when you’re eligible.
- Ask your own questions and get answers to your concerns about the vaccine.
